The electronic portal at Hometown Doc, PA is for established patient use only and for the stated purposes of allowing patients to communicate through electronic means securely – including: requesting office visits, viewing lab results and a patient’s chart. Other functions may be added, and existing functions deleted at any time for any reason. The intent and design are for patients to have access to their own record and no other patient’s record.
Authorized User Responsibilities
Only you can protect and maintain security of your username and password. Patients will be responsible for any action that compromises the security of their own records and any liability that may result. Hometown Doc, PA is not responsible for any access to your information that results from sharing your account information – even with a family member (which may be appropriate if you have designated a relative as “health care proxy”).
Hometown Doc, PA has privacy and confidentiality rules that adhere to the HIPPA guidelines found at https://www.hhs.gov/hipaa/for-professionals/privacy/laws-regulations/index.html. We will comply with all applicable federal and Arkansas state laws that apply to conducting electronic healthcare communications (as detailed in our privacy and security policies followed at Hometown Doc, PA.) Established patients must sign our office policy statement and the same policies apply to our portal and website. By logging into and using our portal, you agree to these terms and conditions. We reserve the right to revoke access at any time for any reason including but not limited to: rude language, use which could overburden our resources (excessive repetition of the same request/issue), or attempting to access another patient’s information w/o permission.
Do NOT use this portal for urgent or emergent requests as significant delays WILL result. Call 911 for truly emergent needs, and our office phone 870-399-0077 during (and after) office hours. Your call may be directed to the physician answering service at North Arkansas Regional Medical Center when the office is closed and the provider is not available. You may call the answering service directly at 870-414-4774 to be put in touch with an on-call physician for any provider.
When you request prescription refills on the portal, please allow 48 hrs. before contacting us again regarding the same refill. You may address issues regarding refills to the pharmacy as well, but duplicate requests will not result in faster service and could result in slower response.
These terms and conditions do not apply to other web sites you may access from our site (such as the one above) which will have different terms and conditions that you should read and understand.

When you provide feedback though our “contact us” links or surveys, you grant us perpetual and irrevocable license to use that feedback for any lawful purpose.
